Requirements include:

• Curriculum Vitae (CV)
• Current Passport. Important Note: TO BE ALLOWED TO ENTER THE COUNTRY, Ecuador requires that the passport's expiration date be at least six months beyond your departure date. Ecuador does not require a visa for stays of up to 3 months. 
• Professional License (for medical personnel)
• A personal reference with contact information
• A signed agreement to the Rules of Conduct
• A signed Release of Liability Waiver
• A signed waiver to use  Photos and Videos for promotional purposes
• A signed Waiver for BMM to seek medical treatment for you if necessary
• An uploaded  "headshot" or casual picture of yourself

Important Note:  If your passport is close to expiration, we suggest immediate action to renew since the current times for renewals of US passports are incredibly long.
